Network Enrollment Made Easier: Here’s What’s New

Applies to UnitedHealthcare Community Plan Home- and Community-Based Services

This update applies to UnitedHealthcare Community Plan Home- and Community-Based Services, or HCBS, providers.

UnitedHealthcare has updated the onboarding process for HCBS health care providers who support UnitedHealthcare Community Plan Medicaid and long-term services and supports, or LTSS, programs.

The new process is designed to reduce misrouted requests, avoid delays, and make provider onboarding clearer.

What has changed

The HCBS participation request process has been updated to make submissions easier to route and review. Key changes include:

  • The request form now matches specific HCBS service types.
  • Requests are automatically sent to the correct onboarding team.
  • Instructions are clearer, which helps reduce mistakes and resubmissions.

These updates help providers submit the right information the first time and allow the appropriate team to review the request faster.

Services covered under this process

The updated onboarding process applies to several HCBS service categories, including but not limited to:

Personal care and in-home support services

This includes:

  • Personal care
  • Attendant care
  • Homemaker services
  • Habilitation
  • Respite care
  • Private duty nursing

Case management

This includes:

  • Supports coordination
  • Related case management services

Residential HCBS settings

This includes:

  • Assisted living
  • Adult foster care
  • Developmentally disabled group homes

Adult day and community-based services

This includes:

  • Adult day care
  • Adult day health
  • Medical day services

Self-direction and core HCBS supports

This includes:

  • Fiscal intermediary services
  • Financial management services
  • Home-delivered meals
  • Personal emergency response systems
  • Assistive technology
  • Home and environmental modifications
  • Transition services

How to request participation

To submit a participation request:

  1. Go to the Join the network – Ancillary providers and centers page.
  2. Select Home- and Community-Based Services, or HCBS.
  3. Complete and submit the online request form.
  4. Save your confirmation after submission.

Following these steps helps ensure your request is routed to the correct onboarding team.

What happens after submission

After your request is submitted, UnitedHealthcare will review your application and issue a decision within 15 business days.

If approved

A representative will contact you with the next steps.

If declined

You will receive an email or letter with the determination.
