Outpatient Radiology Prior Authorization Program Adds New Imaging Codes
UnitedHealthcare is expanding its outpatient radiology prior authorization program starting Sep 01, 2026. As part of this update, the insurer will add new American Medical Association CPT codes for certain advanced imaging services.
The change will apply to UnitedHealthcare commercial plans, Community Plans, Individual Exchange Plans, Oxford Plans, and Rocky Mountain Health Plans. It will not apply to UnitedHealthcare Medicare Advantage or Dual Special Needs Plans.
New CPT Codes Being Added
The following imaging codes are being included in the program:
- 70471 – CT angiography of the head and neck, performed without and with contrast, including postprocessing.
- 75577 – Quantitative or qualitative coronary plaque analysis using augmentative software on CCTA data, with physician interpretation.
When Prior Authorization Is Not Required
These requirements will not apply when advanced imaging services are performed in:
- Emergency rooms
- Urgent care centers
- Observation units
- Inpatient settings
How Providers Can Submit Prior Authorization Requests
- Sign in to the UnitedHealthcare Provider Portal with your One Healthcare ID
- If you don’t have a One Healthcare ID, register now
- From the left-hand tabs, select "Prior Authorizations & Notifications."
